Overview

Following positive feedback about the usefulness of our reports on themes we identified from Prevention of Future Deaths Reports (PFDs) issued by coroners to health and social care providers in 2021 and 2022, we have repeated the exercise this year by looking at over 250 PFDs issued during 2023 in relation to acute hospitals, mental health, social care and ambulance services.
